LETTER OF APPOINTMENT

  1. Position:

We have pleasure in confirming your appointment to the position of Trainee Dent Technician with Dent Doctor. You will be reporting to the Manager of your branch. The key performance areas of this position have been discussed with you and a copy of your job description will be given to you on your date of commencement.

  1. Commencement:

Your employment in your new position will commence on the ___ day of ______________ 20 _____.

  1. Remuneration:

You will be paid in accordance with your level of expertise. There is a scale of 1 – 5:

1. Apprentices R 1,600.00 per month

2. D Grade R 2,200.00 per month + commission

3. C Grade R 2,800.00 per month + commission

4. B Grade R 3,600.00 per month + commission

5. A Grade R 5,400.00 per month + commission

You are currently at number ___on the scale and will therefore be paid R__________.

Should your commencement date be within the monthly period, you will be paid in proportion to the actual time worked.

  1. Hours of work:

Monday to Friday 08h00 – 17h00

Saturday 08h00 – 13h00

You shall work a 45 hour week. A meal interval of 30 minutes and two rest periods of 15 minutes each are given daily. Specific times for these will be given to you by your Manager.

  1. Overtime:

Due to the nature of our operational and business requirements, you may be required to work overtime from time to time. Remuneration will be in accordance with legal requirements – 1.5 times your hourly rate for the number of hours you have worked overtime from Monday to Saturday; 2 times your hourly rate for the hours you have worked overtime on a Sunday or public holiday.

  1. Discretionary Performance Bonus:

An annual bonus is paid in December at the discretion of the Management and Owners of Dent Doctor. This will be communicated to you annually.

  1. Duties:

Although you have been employed in the capacity of a Dent Technician, you may be required to perform other duties that may be reasonably expected of you within Dent Doctor from time to time.

  1. Leave:
  1. Annual leave

You are entitled to 15 working days leave per annum. This leave is to be taken at the convenience and discretion of Management.

  • Sick leave

Sick leave will accrue at the rate of 12 working days per annum subject to a maximum of 36 days in any 3 year cycle. However, Management must be satisfied that you are not in a fit state to perform your duties, and that your condition could not have been avoided by taking reasonable care or precautions on your part.

Sick leave that is in excess of 2 consecutive days must be supported by a medical certificate from a registered medical practitioner. Dent Doctor may request a medical certificate for periods of illness less than 2 days; or any sick leave that occurs either before or after a weekend or public holiday.

  • Special leave

This may be applied for over and above annual leave and is granted at the discretion of Management. This may be granted in the form of paid or unpaid leave.

  1. Confidentiality:

You may not, at any time, divulge to any person any trade secrets or any other confidential information concerning the Company’s business or affairs, or that of any of its associates, without the Company’s written consent.

  1. Other interests/part-time employment:

No employee may be directly or indirectly engaged in any other business or undertaking other than that of the Company, unless specific written consent is obtained from the Company prior to embarking on such interests.

  1. Company rules:

You agree to abide by the Company rules, as amended from time to time. Details are available from Management and you are encouraged to familiarize yourself with these rules and/or procedures.

  1. Discipline and grievances:

You agree to abide by the Company’s disciplinary and grievance procedures. These are in accordance with legislation and are available to you.

  1. Searches:

You agree to submit yourself to searches of yourself and your possessions by a person designated by the Company. Your co-operation in this regard will be greatly appreciated.

  1. Probation:

You will be required to serve a three (3) month probationary period. During this probationary period, 24 hours written notice will apply to either employer or employee.

Once the probationary period is successfully completed, you will be appointed to the permanent staff of Dent Doctor.

  1. Termination of employment:

During the first year of service, service may be terminated by either party giving no less than two (2) weeks written notice. Thereafter, four (4) weeks written notice will apply.

Should you terminate your service without proper notice, you will forfeit your rights to salary not yet paid, as well as leave in excess of statutory leave. The Company may, however, terminate employment at shorter notice should an employee, in the opinion of the Company, commit a breach of the terms and conditions of employment.

Should you be absent from work for any reason, it is your duty to inform the Company. If you are absent for more than five (5) consecutive days without contacting the Company, you will be deemed as having absconded and your employment will be terminated.

  1. Special conditions:

You agree that once you are fully trained as a Dent Technician, and should you leave the employ of Dent Doctor during the first year after your training, that you pay the Company an amount of R85 000.00 to cover the costs of training.

Should you leave the Company’s employ in your second year after having completed your training, you will pay R75 000.00 to the Company. In your third year, the amount you will pay to the Company is R40 000.00.

This amount is payable to the Company before you leave Dent Doctor’s employ.

  1. Acceptance:

You are kindly requested to sign both copies of this letter, thereby confirming your acceptance of our offer of employment. All pages of both copies must be initialed.
